When the Heat Makes You Dizzy
Every summer Texans are faced with the decision to go outside and face the heat, or stay in the air conditioning. As a vestibular physical therapist, I spend my days helping people whose balance and dizziness systems haven’t been operating as well as usual, and heat is one of the causes that has been coming up more recently. The good news is that once you understand what heat is doing to your body, most of it makes sense and much of it is preventable.
Dizziness is one of the body's earliest heat warning signs
Dizziness, lightheadedness, and that "I need to sit down" feeling are among the first symptoms your body produces when it is struggling to cope with heat. They can show up long before the more dramatic signs of a true heat emergency. That is exactly why they are worth paying attention to: your balance system is often the canary in the coal mine.
Let me walk you through the different ways heat creates dizziness, from the everyday to the dangerous.
1. The blood pressure problem (why you feel faint when you stand up in the heat)
This is the most common form of heat-related dizziness, and it comes down to plumbing. When you get hot, your body sends blood toward your skin so heat can escape. Those blood vessels widen, and blood can pool in your legs. At the same time, sweating drains fluid out of your bloodstream. The result is less blood returning to your heart, lower blood pressure, and briefly less blood flow to your brain, which your brain registers as lightheadedness or even a faint.
This is essentially what "heat syncope," or heat-related fainting, is. It classically happens when someone stands for a long time in the heat, or stands up quickly after sitting or bending. It is especially common in people who are not yet used to the heat, such as during the first several days of a heat wave or the start of a sports season, and in people who take certain blood pressure or fluid ("water") pills. The dizziness here is a circulation problem, not an inner-ear problem, but the sensation can feel very similar.
2. Dehydration and fluid loss
Heavy sweating without enough fluid replacement lowers your blood volume and throws off your electrolytes (the salts your nerves and muscles depend on). This magnifies the blood pressure problem above and adds fatigue, headache, nausea, and weakness. Dehydration is a major reason people feel "off balance" and foggy in the heat even when they never come close to fainting.
3. Heat exhaustion
When heat strain builds up, you can cross into heat exhaustion. Typical symptoms are profound fatigue, weakness, heavy sweating, headache, nausea, and, very commonly, dizziness. The body temperature may be mildly elevated but is generally below 104°F, and importantly, the person is still thinking clearly. Heat exhaustion is your body waving a large flag telling you to stop, get out of the heat, cool down, and rehydrate. Taken seriously, it usually resolves quickly. Ignored, it can progress.
4. Heat stroke (a medical emergency)
Heat stroke is the most severe and it is life-threatening. It is defined by a very high core temperature (over about 104°F) combined with clear changes in brain function. Dizziness is one of the early neurologic signs, but it comes alongside confusion, slurred speech, agitation, poor coordination, and sometimes collapse or seizures. The single most important difference between heat exhaustion and heat stroke is mental status: if someone in the heat becomes confused, disoriented, or is acting strangely, treat it as an emergency.
Call 911 immediately, move the person to a cool place, and begin aggressive cooling (cold water, ice packs to the neck, armpits, and groin) while you wait for help. Rapid cooling saves lives and brains.
5. The part most people (and even some clinicians) don't know about: heat and the balance system itself
Here is where my vestibular lens comes in. Heat does not just mess with your blood pressure. It can directly affect the sensors and nerves your body uses to stay balanced.
- Proprioception takes a hit. Research shows that when the body overheats, the signals traveling to and from your muscles are altered, which measurably worsens balance and body-position awareness. In plain terms, being overheated makes your feet and joints a little less reliable at telling your brain where you are in space, which can feel like unsteadiness.
- Dehydration plus heat worsens balance control. Studies of people exercising in hot conditions while dehydrated show poorer postural control and neuromuscular control afterward, which translates to a higher risk of missteps and falls.
- Temperature affects the inner-ear balance nerves. In laboratory studies, the firing of the balance (vestibular) nerves changes with core body temperature. Your inner ear is normally well insulated, but the nervous system that processes balance is temperature-sensitive.
- Severe heat can, rarely, injure the brain's balance center. There are documented cases of people developing true vertigo about a week after significant heat exposure, with findings pointing to injury of the vestibulocerebellum, the part of the brain that coordinates balance and eye movements. This is uncommon, but it is a reminder that serious heat illness can have lasting effects on the balance system, and that dizziness which lingers after a heat event deserves a proper evaluation.
When heat-related dizziness is more than "just the heat"
Most heat dizziness fades once you cool down and rehydrate. See a professional if:
- Dizziness or vertigo persists for days after a hot day or a heat illness
- You have spinning vertigo, double vision, trouble focusing your eyes while moving, or new unsteadiness when walking
- You have fainted, or nearly fainted, more than once
- You have a heart condition or take blood pressure or diuretic ("water") medications and notice new dizziness in the heat
Lingering, movement-provoked, or visually-triggered dizziness is exactly what vestibular physical therapy is designed to treat. Through gaze-stabilization exercises, balance and gait training, and gradual, structured exposure to the movements that provoke symptoms, we can retrain the balance system and reduce both dizziness and fall risk.
How to keep safe in the summer
- Hydrate before you're thirsty, and include electrolytes during long or sweaty activity.
- Acclimatize gradually. Ease into hot-weather activity over one to two weeks rather than going all-out on day one.
- Change positions slowly. Stand up in stages, especially after sitting, bending, or lying down.
- Take cooling breaks in the shade or air conditioning, and don't push through early warning signs.
- Avoid the hottest part of the day for hard exertion when you can.
- Know your medications. Ask your doctor or pharmacist whether yours affect blood pressure, fluid balance, or heat tolerance.
- Respect the warning signs. Dizziness in the heat is information. Stop, cool down, and rehydrate before it becomes something worse.
Heat-related dizziness sits at the crossroads of your circulation, your hydration, and your balance system, which is why it is so common and why it responds so well to simple, proactive steps. Listen to that early lightheadedness. It is your body's way of asking for a break before the heat gets the better of you.
